The Guidelines are Changing!

New guidelines released by the American College of Sports Medicine (ACSM) have changed the fitness landscape dramatically. The notion that you have to make a “lifestyle change” in order to get results from exercise is a thing of the past. Small changes in physical activity can dramatically change your risk for increased disability.

When it comes to cancer, in the recent past it was a common belief among medical professionals that you shouldn’t exercise at all, that you should let your body rest and focus all it’s energy on recovery and healing. The science is clear on this: the exact opposite is true, exercise helps boost your body’s ability to fight cancer. It was also previously believed that you had to exercise continuously for 10 minutes or more to get the cardiovascular benefits of aerobic exercise. More recent research shows that you will get a health benefit even from very short bouts of physical activity (PA) that increase your heart rate above resting.

Each time you increase your PA over your resting state beyond your regular routine, you have the opportunity to improve your health. There is no need to think of making big life changes as a necessary means to achieving beneficial health goals; small incremental additions to your PA will start to show results over time.

What Small Steps Can I Take?

All you need to do is start increasing your daily physical activity to reduce your risk of disability associated with androgen deprivation therapy, radiotherapy or chemotherapy. Walking up and down your street or a set of stairs an extra time or two is considered an increase to your PA. If you add small bouts of increased activity in your daily routine, you can create a significant increase in your total daily output. The worst thing you can do is become less active.

Some very small examples are getting to those dishes that need to be washed, doing regular household chores and especially getting out and watering your plants or doing some gardening that has been neglected. All of these things will increase your daily PA, improve your quality of life and reduce your risk of disability.

You may not feel like this next recommendation is a small step but if you can do it, you will see the most significant improvement in your health. Going to the grocery store, cooking your own food and doing the dishes instead of eating out on a regular basis is a great way to increase your daily PA and reduce your overall caloric intake. Restaurants don’t have your health in mind, they are trying to give you the best tasting food with the least effort on your part. Just doing this ritual for most of your meals throughout the week can have a tremendous benefit in your life.

The Leap to Exercise…

Once you have added these small steps up they often lead you to wanting to do more. That’s when you start adding regular exercise to your daily schedule. Starting with walks and hikes or maybe a bike ride on a sunny evening. Remember, exercise doesn’t have to be painful, in fact, it shouldn’t be. I find the best way to keep consistent is to do activities that I find enjoyable that also get my heart rate up.

Make time for the things you love, especially if they increase your heart rate and utilize a little muscle. Your body and mind will love you back for it. This is the road to living your active passion.

This is an adaptation of an exercise in the POWIR series of exercises created by Dr. Kerri Winters-Stone for her research on Osteoporosis. These exercises have been repeated in many other studies and have proven to be effective at increasing Bone Mineral Density (BMD), minimizing the loss of BMD and increasing strength.

The Band Single Arm Standing Row should be first done with the lowest amount of resistance possible. Once you have become proficient in the full range of motion you can increase resistance. In this example I am using a 40″ Superband. Researchers used bands and cable row machines in a seated position; however, dumbells, barbells, kettlebells, sand bags and a standard resistance band with handles are all suitable. I prefer the superband, which is featured in the video, it is versatile and can shortened to increase resistance as you will see in subsequent exercises.

  • Start by placing one end of the band beneath your foot with your hips forward and your head up.
  • As you pull up from the startig position, drive your shoulders from the front leaning to upright position, concentrating on generating the power of this movement from your waist and back.

  • You will have reached the absolute top of the lift when your glutes are tight and your hips are in a neutral position. Avoid locking your knees.
  • Drop your hips and shoulders to initiate the next rep, keeping your weight evenly distributed between the two.
  • Do not lift with your lower back. You will feel the exercise here but concentrate on driving through the arch of your foot on the up phase and collapsing your body evenly on the down phase.

The Workout

The POWIR4ADT core workout is a series of exercises adapted to men with prostate cancer. The original study was conducted on healthy, premenopausal women in order to research site specific response to resistance training on bone mineral density. Because there ise an age difference and health issues that would affect the two populations differently, we have adjusted the exercises appropriately to older men undergoing treatment for prostate cancer.

BEFORE YOU START

We have exercise pre-screening tools availble on this site for you to review before starting any exercise program. Please utilize these tools and consult with a physician if necessary before begining any new fitness program.

You don’t necessarily need any equipment, you can do it all with just body weight to start. Once you are confident with your form and don’t feel any pain with full range of motion, then it is advised to add more resistance. 

We recommend resistance bands. They are inexpensive, easy to transport and take with you wherever you go and are great for joint stability. But, as with all of our exercises, we want to remove roadblocks from your physical activty so feel free to use whatever form of resistance you have readily available to you.

The First Exercise Block

Block one is a standing shoulder series that can be done anywhere without any equipment at all. This series is taken directly out of the U.S. Army Physical Training handbook. 

This is a great series to improve shoulder strength and get you started on planks and push-ups as well as other shoulder and chest exercises that are a part of the POWIR4ADT core exercise group.

These exercises have been repeated in many other studies and have proven to be effective at increasing Bone Mineral Density (BMD), minimizing the loss of BMD and increasing strength.

The Band Bent Leg Deadlift should be first done as a bodyweight only exercise. Once you have become proficient in the full range of motion you can advance to resistance. In this example I am using a 40″ Superband. Researchers used weighted vests; however, dumbells, barbells, kettlebells, sand bags and a standard resistance band with handles are all suitable. I prefer the superband, which is featured in the video, it is versatile and can be doubled up to increase resistance as you will see in subsequent exercises.

  • Start by doubling up the band and placing the folded band beneath your feet with your hips down and your head up.
  • As you move up from the startig position, drive your shoulders from the front leaning to upright position, concentrating on generating the power of this movement from your waist.

  • You will have reached the absolute top of the lift when your glutes are tight and your hips are in a neutral position. Avoid locking your knees.

Simple

I’ve never been into complicated leg extension and pull down machines, I like my equipment to be simple and not need to be plugged in or maintenanced on a regular basis.

There are 3 items in my fitness equipment arsenal that I use most: the kettlebell, suspension trainer and super bands. Everything but push pull and curl exercises can be done bodyweight. So you don’t need much beyond your own body to have an unlimited supply of exercises at your fingertips.

There are lots of companies that sell bands but I recommend when you buy online to purchase Spri. They are consistently quality bands and will last. Other, off brand names may be cheaper but are likely to snap on you.

Indoor/Outdoor

I like to train outside whenever the weather permits. Metal doesn’t like to go with me. I avoid dumbells and barbells because they are bulky, require a lot of sets to be effective and aren’t great for taking outside. Suspension trainers, bands and sand bags all are highly mobile, don’t rust and are great outdoors on a sunny day. Plus, to change the resistence all you need to do is adjust your grip, your angle or add/subtract some sand. 

Convenient

We won’t train if getting setup isn’t convenient. Motivation is a big part of keeping a consistent training schedule. If I feel like there is going to be a lot of setup time and it will cut into the short time I have to workout, I tend to skip. I’ve learned this about myself so I keep it simple and convenient. I have two kettlebells, one for swings and squats and another for lifts, 3 sets of super bands, light to heavy and a suspension trainer. I can take this small kit anywhere and it takes two trips to get it from the truck or room to where I’m going to train. It would be one trip but the squat kettlebell is 44kg.

Versatile

When I purchase a piece of equipment, especially for online training, I make sure that it has more than one use. The more exercises I can do with it the better. This is important for two reasons, one, intensity. If I can use a sand bag for squats, rows and presses, then I can create a high intensity 45/45/45 interval that gets my heart rate up without switching between machines or weights. The same for my clients in an online appointment. If I can use super bands and move from a curl to a front press, then tricep extension, I’m keeping the intensity up and they are getting a great workout. Versatility is key when choosing equipment for online or at home training.

Basic is Good

Don’t confuse basic for cheap. In fitness, you get what you pay for, especially when you purchase online. I avoid complicated pieces of equipment that tout a full body workout. If I can see how I can lift it, press it or squat with it without an instructional booklet, then it is probably basic enough for me. Sand Bags and Super Bands are two of the most basic pieces of equipment that don’t need much instruction to know how to use. A good trainer will unlock myriad exercises with equipment like this that will hit every muscle in your body.

Sand bags or water bags are great. They travel well and are extremely versatile. The shifting weight is great for core stabilization and the different handle holds allow fo a nearly unlimited selection of exercises.

Don’t Go Cheap

Everything I recommend isn’t inherently expensive but I wouldn’t try to find the cheapest version of it either. Bands made of good, consistent latex are essential. Kettlebells with nice grips and good handle shape will change how you train. Suspension trainers with good clips and strong carabiner will give you a  better sense of safety. Going cheap, at best, provides a terrible workout experience, at worst, could be dangerous if a band snaps or a carabiner brakes. I can’t emphasize this more, always buy from a reputable online retailer or go to your local fitness equipment supplier and test out the equipment before you purchase.

Equipment Essential Traits

 

  • Versatile: can be used for multiple exercises and muscle groups.
  • Quality: make sure that you aren’t buying the cheapest. Grips, materials and tensile strength are key to safety and good exercise experience.
  • Mobility: make sure your equipment travels well. You want to be able to take your gym on the road.
  • Weather proof: Outdoor training is more beneficial than being indoors. Make sure you can take your equipment outside.

Cost Effective

There are many people that won’t work out unless someone holds them accountable. Enter the personal trainer. I have created a ton of programs to reduce the cost to my customers, providing a supplementary aerobic schedule, giving them a workout of bodyweight HIIT exercises they can do at home, etc. But they will only do the workout when they have scheduled the appointment with me. Seeing a trainer 3 times a week or more for 45-60 minutes a pop can be expensive. Que the virtual personal training appointment. An online appointment can be as short as 15 minutes and could be a quick HIIT workout then sending you on a 30 minute walk or a 15 minute run. Innovative

Training online isn’t better or worse than in-person, it is different. It makes you use a different set of fitness tools and flex some different grey matter. There’s not a lot of time for chit-chat in an online appointment so they move fast and I often use more basic equipment like kettlebells, bands and bodyweight exercises. My virtual personal training appoints by nature are just different than what I do in-person and are a great change up in my client’s programming.

There’s something about training in a studio and meeting a client online at their office or home that makes the sessions more personal. The professional relationship between a client and personal trainer is often strong, simply because your trainer is providing one of the most important things in your life; health.
